Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Prepare nutritious meals tailored to individual dietary needs and preferences
- Monitor vital signs and observe patient conditions to identify any changes or concerns
- Administer medications accurately following care plans and HIPAA privacy regulations
- Maintain a clean, safe environment through regular cleaning and organization of living spaces
- Support clients with memory care needs such as Alzheimer’s or dementia, implementing specialized techniques
- Use equipment like Hoyer lifts or other assistive devices to ensure safe transfers and mobility support
- Document patient observations, care activities, and updates in compliance with healthcare standards
- Collaborate with healthcare professionals to develop and follow personalized care plans
- Provide emotional support and companionship to foster a positive environment for residents or clients
Skills
- Proven experience in home & community care, assisted living, or long-term care environments
- Knowledge of patient monitoring, vital signs, medication administration, and HIPAA regulations
- Familiarity with Alzheimer’s, dementia, autism spectrum disorders, and hospice or palliative care approaches
- Ability to perform heavy lifting safely and operate assistive devices such as Hoyer lifts
- Strong observational skills for patient safety and health status updates
- Excellent communication skills for working effectively with clients, families, and healthcare teams
- Certification in CPR and First Aid; additional certifications in nursing or social work are advantageous
- Experience working with individuals with developmental disabilities or disabilities in general
- Ability to follow detailed care plans while maintaining flexibility to adapt to changing needs
