Position Summary
The Advanced Practice Registered Nurse (APRN‑CNP) delivers comprehensive behavioral health and primary care services within an outpatient setting. This role includes conducting diagnostic assessments, developing individualized treatment plans, providing therapeutic interventions, and managing medications—including psychotropic medications and medication‑assisted treatment (MAT). The APRN may also supervise non‑licensed staff or nursing personnel as assigned and collaborate closely with multidisciplinary team members to ensure high‑quality, integrated care.
Key Responsibilities
Behavioral Health & Addiction Treatment
- Conduct holistic assessments addressing medical, psychological, emotional, and social needs.
- Collaborate with clients to develop individualized treatment plans for mental health and/or substance use disorders.
- For clients receiving addiction services, determine or collaborate with the Clinical Supervisor on the appropriate ASAM level of care.
- Prescribe and manage psychotropic medications when board‑certified as PMHNP‑BC.
- Provide medication‑assisted treatment (MAT) for eligible clients following completion of required training.
- Coordinate multidisciplinary services for clients with complex behavioral health needs.
- Refer clients to community resources, higher levels of care, mental health providers, and medical specialists as appropriate.
- Document all services thoroughly and within agency timelines in the electronic health record.
- Meet agency productivity standards and participate actively in multidisciplinary team meetings.
- Maintain compliance with federal, state, legal, and contractual regulations, including mandated reporting.
- Uphold HIPAA and 42 CFR Part 2 confidentiality requirements.
Primary Care Responsibilities
- Conduct basic health screenings, including vitals, physical assessments, and preventive care evaluations.
- Diagnose and manage common acute and chronic medical conditions within the APRN scope of practice.
- Provide health education, wellness counseling, and disease‑prevention guidance.
- Order and interpret laboratory tests, imaging, and diagnostic studies.
- Prescribe medications for primary care conditions in accordance with Ohio APRN regulations.
- Identify medical concerns requiring specialty care and initiate appropriate referrals.
- Monitor and manage chronic conditions such as hypertension, diabetes, asthma, and other primary care needs when appropriate.
- Collaborate with behavioral health staff to ensure integrated medical‑behavioral treatment planning.
Professional & Administrative Duties
- Maintain an active Standard Care Arrangement (SCA) with a collaborating physician.
- Follow all agency policies, procedures, and quality‑assurance standards.
- Participate in chart audits and quality‑improvement activities.
- Demonstrate professionalism and uphold the values of Freedom to Recover Counseling & Treatment Center.
- Perform additional duties as assigned.

Education & Experience Requirements
- Master of Science in Nursing or equivalent graduate degree.
- Certified Nurse Practitioner; PMHNP‑BC preferred.
- Minimum of two years of relevant clinical experience.
- Active, unencumbered Ohio APRN license.
- Active DEA registration.
- Board certification through AANP, ANCC, NCC, or similar body preferred.
Physical Requirements
- Prolonged periods of sitting and computer work.
- Periods of alternating sitting, standing, and bending during patient assessments.
Pay: $55.00 - $70.00 per hour
