Duties
- Conduct comprehensive patient evaluations to determine physical therapy needs, including assessments of physiology, anatomy, and neurological function
- Develop and implement customized treatment plans targeting orthopedic, neurological, pediatric, and geriatric patients
- Collaborate closely with healthcare professionals such as physicians, nurses, and occupational therapists to coordinate care plans and optimize patient outcomes
- Manage discharge planning processes by preparing detailed reports and recommendations for ongoing care or community services
- Document all patient interactions accurately in electronic medical records (EMR) systems following clinical documentation standards
- Provide patient education on injury prevention, treatment exercises, and home care strategies to support rehabilitation outside clinical settings
- Interest in neurological conditions would help grow this role within our practice
Requirements
- Valid state licensure ND and MN as a Physical Therapist with practicing rights in relevant healthcare settings
- Demonstrated experience in outpatient clinics or home health services with a focus on patient rehabilitation
- Strong knowledge of physiology, anatomy, medical terminology, and clinical documentation standards
- Proven ability to develop effective treatment plans for diverse populations including geriatrics, pediatrics, orthopedics, and neurology patients
- Experience working with electronic medical records (EMR) systems for accurate documentation
- Excellent client communication skills to educate patients and collaborate with multidisciplinary teams
- Ability to perform discharge planning efficiently while adhering to healthcare protocols
- Familiarity with outpatient rehabilitation center practices and inpatient hospital procedures is preferred
