Duties
- Conduct comprehensive patient assessments to determine functional limitations and develop tailored treatment plans aligned with clinical documentation standards.
- Implement therapeutic interventions including manual therapy, trigger point therapy, and exercise programs to facilitate patient rehabilitation across orthopedic, neurological, pediatrics, and geriatrics specialties.
- Monitor patient progress regularly, adjusting treatment strategies as needed to maximize recovery outcomes.
- Collaborate closely with healthcare professionals such as physicians, nurses, and occupational therapists to coordinate care plans and discharge planning efforts.
- Maintain accurate and detailed electronic health records (EHR) and electronic medical records (EMR) systems, ensuring compliance with documentation standards.
- Educate patients and their families on home & community care strategies to support ongoing recovery outside clinical settings.
- Assist with discharge planning by evaluating patient needs for continued therapy or community-based services, ensuring seamless transitions from inpatient or hospital environments to outpatient or home health care.
Skills
- Strong knowledge of physiology, anatomy, and medical terminology relevant to physiotherapy practice.
- Proven experience working with outpatient rehabilitation centers, hospitals, or inpatient settings with a focus on outpatient clinic experience.
- Skilled in developing comprehensive care plans that incorporate patient goals and clinical best practices for treatment plan development.
- Excellent client communication skills to effectively educate patients about their treatment options and progress.
- Familiarity with outpatient experience including outpatient rehabilitation center operations and home health environments.
- Ability to work collaboratively with healthcare professionals across disciplines for integrated patient care.
- Proficiency in managing electronic health records (EHR) systems and EMR systems while adhering to clinical documentation standards.
- Experience working with diverse populations such as geriatrics, pediatrics, orthopedics, neurology, and patients requiring trigger point therapy or physiotherapy interventions.
- Knowledge of discharge planning processes and acute care experience enhances your ability to deliver comprehensive patient care.
