Duties
- Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor patient health by observing vital signs such as blood pressure, temperature, and pulse; report changes promptly
- Administer medications accurately following medical instructions and maintain detailed medical documentation
- Support patients with specialized care needs such as Alzheimer’s, dementia, hospice, and end-of-life care
- Operate Electronic Health Record (EHR) systems to document patient interactions and medical records securely in compliance with HIPAA regulations
- Use patient transfer methods safely, including Hoyer lifts and other assistive devices, ensuring proper technique to prevent injury
- Perform light housekeeping tasks like cleaning, laundry, meal preparation, and maintaining a safe environment
Experience
- Previous experience working in assisted living facilities, nursing homes, or home healthcare agencies preferred
- Proven ability to work with geriatric patients and individuals with developmental disabilities or disabilities requiring specialized care
- Familiarity with medical terminology, catheter care, wound care, and basic life support (BLS) or CPR certification required
- Experience using EMR (Electronic Medical Records) systems for documentation and patient tracking preferred
- Knowledge of home health care regulations and best practices for patient confidentiality under HIPAA guidelines
- Demonstrated ability to communicate effectively with clients and healthcare teams to ensure comprehensive patient care
- Prior live-in employment experience or long-term caregiving roles is advantageous
