Proactive Clinical Partners is seeking a Nurse Practitioner to provide care in skilled nursing facilities in the Columbus area. Our model allows providers to work independently while collaborating closely with physicians to deliver high-quality care to geriatric patients.

What You'll Do
- Evaluate and manage patients with complex health conditions

Requirements
- Active Indiana Nurse Practitioner license
- Experience in acute, post-acute, or skilled nursing preferred
- Strong communication and organizational skills
- Ability to work independently
