Position: LPN – Continuing Care
Location: Grand Rapids, MI
Start Date: Sep 8, 2026(Tentative)
Schedule: 6:30pm -7am (36 hours/week).
Weekend rotation: Every other weekend
On Call: None
Pay Package (36 hours/week): $1840 weekly total
W2: $15/hour
Travel, Lodging and Meals (Non- Tax): $1300/week
Requirements:
- Active MI LPN License.
- BLS Certification (AHA or Red Cross).
- Min. 2 years of experience in LTC/SNF.
- Rehab experience, familiarity with EPIC, trachs, admissions, high pace and task completion. Worked in a SNF setting. Experience with feeding tubes, and neuro patients.
