Come join the Hillendale Home Care team! We're looking for a dynamic Home Care Community Partner to join our growing Walnut Creek team.
The Home Care Community Partner is first and foremost a business development role focused on cultivating referral relationships, expanding strategic partnerships, and maintaining a visible presence throughout the market. The remaining time is focused on referral coordination, community engagement, and relationship stewardship that supports long-term business growth.
Primary Responsibilities
Business Development & Referral Relationships (Approximately 60-70%)
- Develop and maintain referral relationships with hospitals, skilled nursing facilities, senior living communities, physicians, fiduciaries, elder law attorneys, care managers, and other community partners.
- Conduct consistent field visits to build relationships, uncover opportunities, and maintain Hillendale's visibility.
- Develop and manage a pipeline of strategic referral relationships.
- Identify new partnership opportunities throughout the Walnut Creek market.
- Represent Hillendale at networking events, community organizations, educational programs, and industry functions.
- Develop account plans for strategic referral partners and consistently follow through on relationship-building activities.
- Confidently ask for referrals and new business opportunities while maintaining Hillendale's consultative, relationship-first approach
Referral Coordination & Family Engagement (Approximately 10-15%)
- Respond promptly to referral inquiries with professionalism and empathy.
- Help families understand Hillendale's services and coordinate the appropriate next steps.
- Partner closely with Intake and Client Care Managers to ensure a seamless transition into the assessment and admission process.
- Participate in hospital bedside meetings, senior living introductions, and referral-related meetings when appropriate.
- Document referral activity, follow-up, and outcomes within Hillendale's CRM.
Community Engagement (Approximately 10-15%)
- Develop educational programs, Lunch & Learns, appreciation events, and other community initiatives that strengthen referral relationships.
- Partner with senior living communities and healthcare organizations to create meaningful engagement opportunities.
- Represent Hillendale as a trusted community resource while increasing visibility throughout the Walnut Creek market.
Relationship Stewardship (Approximately 10-15%)
- Maintain regular communication with referral partners following admissions to reinforce trust and strengthen long-term relationships.
- Conduct occasional client or family visits when appropriate to reinforce relationships and support future referral opportunities.
- Collaborate with Client Care Managers, Intake, Staffing, and Operations to ensure referral partners receive timely communication and an exceptional experience.
Job Requirements
- Three or more years of successful experience in business development, healthcare sales, physician relations, senior living, home care, hospice, community outreach, or another relationship-driven role.
- Demonstrated ability to develop professional relationships that generate measurable business growth.
- Excellent communication, presentation, and interpersonal skills.
- Strong organizational skills with exceptional follow-through.
- Ability to work independently while managing a field-based territory.
- Experience using CRM software and Google Workspace.
- Valid CA driver's license and reliable transportation.
- Understanding of HIPAA requirements and the ability to maintain strict confidentiality.
- Consultative selling approach grounded in trust and professionalism.
- Comfortable asking for referrals and business opportunities.
