Primary Responsibilities
The Mobile Primary Care Nurse Practitioner will:
- Provide comprehensive primary care to patients in their homes and other community-based settings.
- Evaluate, diagnose, and treat acute and chronic medical conditions.
- Perform comprehensive physical examinations and health assessments.
- Develop individualized treatment plans and follow-up plans.
- Order and interpret appropriate laboratory and diagnostic studies.
- Prescribe medications and manage medication regimens within the scope of practice.
- Provide preventive care and health education.
- Manage chronic conditions and help reduce unnecessary emergency room visits and hospitalizations.
- Coordinate care with other healthcare professionals and services when appropriate.
- Accurately document patient encounters and clinical decisions in the EHR.
- Communicate effectively with patients, families, caregivers, and members of the healthcare team.
- Participate in a collaborative, team-oriented approach to patient care.
- Utilize available technology, EHR systems, digital platforms, and AI-assisted tools appropriately and responsibly.
- Maintain professional standards, confidentiality, and compliance with applicable healthcare regulations.
- Travel to patient residences throughout the assigned service area.
Qualifications & Requirements
Required:
- Current Nurse Practitioner license and certification appropriate for primary care practice.
- Excellent clinical judgment and critical-thinking skills.
- Excellent verbal and written communication skills.
- Bilingual in English and Spanish.
- Strong interpersonal skills and the ability to establish trust and rapport with patients and families.
- Compassionate, professional, and patient-centered approach.
- Comfortable working independently while also functioning effectively as part of a healthcare team.
- Comfortable using computers, electronic health records (EHR/EMR), and other healthcare technology.
- Willingness and ability to learn and utilize AI-based healthcare and administrative systems.
- Valid driver's license and clean driving record.
- Ability to successfully pass a background check.
- Reliable transportation and willingness to travel to patient homes.
- Ability and willingness to work outdoors and in varying weather conditions, including occasional inclement weather, as required by the mobile nature of the position.
Preferred:
- Previous experience in primary care, geriatrics, home-based medicine, urgent care, or a similar setting.
- Active DEA registration.
- Experience caring for elderly, medically complex, or homebound patients.
- Experience with EHR/EMR systems and technology-driven healthcare environments.
