Hospice Liaison - Registered Nurse (RN)
Calling all compassionate and driven Registered Nurses (RN)! We’re looking for an experienced Hospice Liaison RN to join our fast-paced and highly respected team at Catholic Health Good Shepard Hospice.
In this impactful role, you’ll be the bridge between patients, families, hospitals, and hospice care — helping guide individuals through one of life’s most important journeys with compassion, dignity, and support.

What you’ll be doing:
- Meet with patients and families to discuss hospice care options
- Coordinate referrals and hospital discharges
- Build strong relationships with physicians, hospitals, and referral sources
- Advocate for patients and families while promoting the benefits of Catholic Health services
- Help grow and expand our hospice program within the community
What we’re looking for:
- Active NYS RN License (MSW candidates also encouraged to apply)
- Strong communication and relationship-building skills
- Hospice or Home Care experience preferred
- EMR and computer proficiency

Salary Range:
USD $100,000.00 - USD $115,000.00 /Yr.
This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job, and it does not include the value of benefits.
