Brownstone Behavioral Health Services Crisis Stabilization program is a mental health support service, seeking a motivated Home Health Aide capable of working independently and in group settings. Must be detail-oriented, have strong verbal and written communication skills, highly organized, and committed to creating a healthy environment to meet patient’s needs. The Home Health Aide should, in their relationship with clients and other staff, model healthy, supportive behavior, and good coping skills. Prior Home Health Personal Care is a plus.
Responsibilities:
- Support daily living & skill development by instructing and assisting residents in activities of daily living, including personal hygiene, nutrition, socialization, and general life skills.
- Maintain clinical documentation by creating and maintaining accurate records, including daily progress notes, individualized service plans, and discharge summaries.
- Maintain safety & cleanliness by conducting daily checks of resident rooms and common areas for safety hazards and cleanliness.
- Address household maintenance needs by replenishing supplies, reporting & addressing household maintenance repairs and tasks as assigned by the Household Manager.
- Administer medications, accurately log all medications administered in the daily medication log.
- Attend mandatory staff meetings and annual BBHS staff retreat.
- Facilitate communication & conflict resolution, assist client with connecting to community resources or programs, and act as the primary contact person for the house during assigned shifts.
- Support program operations by carrying out specific duties, participate in recreational activities with residents, as assigned by the House Manager.
- Uphold professional standards, represent BBHS positively and professionally to residents, their families, and external organizations in all interactions.
- Document all relevant incidences and observations, and brief next staff at shift turn over.
- Respond to emergencies, provide immediate crisis intervention, follow established protocols to deescalate situation, perform CPR, and contact emergency services & program leadership when necessary.
