The Direct Care Staff will perform all duties outlined in this job description. The Direct Care Staff is an extension of the Human Resource Manager and will provide direct support to the clients we serve. The Direct Care Staff will work in concert with the Human Resource Manager to ensure that internal policies and external regulations are adhered to.
Reporting Relationship
The Direct Care Staff reports to the Human Resource Manager.
Essential Duties and Responsibilities: Client Support:
- Administer client medications and complete required documentation (in adaptive)
- Treat clients with dignity and respect in all interactions Incorporate a client-centered philosophy
- Accept delegation of various responsibilities per the Management Team.
- Mediate client crisis situations.
- Ensure that the house program schedule is being followed accurately and closely
- Provide client supervision per the client's Crisis plan/ Crisis Input plan and make sure each plan is being followed
- FAMILIARIZE YOURSELF WITH ALL CRISIS SITUATIONS, PROGRESS NOTES, APPOINTMENT CLOSURES (FROM THE PREVIOUS DAY), APPOINTMENTS (FOR TODAY), AND ACTIVITIES FOR THE DAY AT THE BEGINNING OF YOUR SHIFT (IT IS YOUR JOB TO MAKE SURE THAT THE RESIDENT IS PREPARED AND IS GIVEN THE BEST POSSIBLE CARE)! ! !
- Familiarize yourself and notify your residents about their appointment a day prior to the appointment (this will make sure they will not miss an appointment), you are also required to have the resident ready 30mins before their appointment
- Make sure that all required forms are being completed, including incident reports, home pass, fire drill, direct daily care, progress notes, etc. (in adaptive)
- Document every 2 hours in each resident's progress notes, you will have completed 4 progress notes by the end of your shift (you will be required to know where the resident is, when they left, when they are coming back, what they did, and document this in your notes)
- Preform the following whenever necessary, 3rd shift(do laundry, fold clothing, hang clothing, and consolidate them in the living room), I Et shift (take all fresh laundry form the living room and hang them in the appropriate resident's closet, pick up all clothing on the floor and hang/ fold it in the appropriate places), 2nd shift (take all dirty laundry from each resident's room, compile the laundry in the living room for the 3rd shift- keeping all resident's laundry separate, and communicate laundry specifics to the -yd shift).
- 2nd shift Maintain each resident's room in good standing (nothing on the floor, beds made, free of dishes, night stands free of clutter, etc.) Perform all ILS activities.
- Document in the House communication book daily
- Read progress notes, messages, appointment closures, and medication changes that will be documented in Adaptive or the House Communication book
- Follow all medication procedures, If there are any mistakes or questions call the residential coordinator before administering

Pay: $15.25 - $16.25 per hour
