We are currently seeking a full-time Physical Therapist (PT) to join a dynamic team providing services in both outpatient and home health settings. Key Responsibilities

Perform comprehensive evaluations of patients to assess strength, mobility, balance, coordination, and functional ability.
- Develop and implement individualized physical therapy treatment plans based on clinical findings and physician orders.
- Deliver evidence-based interventions including therapeutic exercise, gait training, manual therapy, pain management techniques, and modality use.
- Document all evaluations, treatments, and patient progress in compliance with regulatory, facility, and professional standards.
- Collaborate with nurses, physicians, occupational therapists, speech-language pathologists, and other healthcare professionals to ensure coordinated care.
- Communicate effectively with patients and families regarding treatment plans, progress, and home exercise programs.
- Comply with all federal, state, and facility policies, including HIPAA, Medicare/Medicaid guidelines, and infection control protocols.
- Participate in team meetings, in-services, and quality improvement initiatives as needed.
Qualifications
Required

Graduate of an accredited Physical Therapy program (Bachelor’s Degree required; Master’s Degree preferred).
- Current or eligible state Physical Therapy license.
- Strong communication, clinical reasoning, and documentation skills.
