Position Summary
The LPN will play an essential role in providing nursing care, administering medications, and ensuring the health and well-being of individuals in residential programs. You will work closely with a multidisciplinary team to support and enhance the quality of life for residents.
Key Responsibilities
- Administer medications and treatments as prescribed
- Monitor individuals’ health conditions and report changes
- Provide direct nursing care and assist with daily living activities as needed
- Support individuals with IDD/BH in maintaining health and safety
- Document all care provided and maintain accurate health records
- Educate and train direct support staff on health-related tasks
- Collaborate with RNs, Nurse Trainers, and other healthcare professionals
- Ensure compliance with PA ODP regulations (6100/6400) and other applicable standards
- Promote a supportive, person-centered environment for individuals
Qualifications
Required:
- Active LPN License in Pennsylvania
- Experience in IDD, Behavioral Health, Home Health, or Residential Care
- Knowledge of medication administration and clinical documentation
- Compassionate and patient-centered approach to care
- Strong communication and teamwork skills
- Ability to work in a residential setting and travel between homes as needed
Preferred:
- Familiarity with PA ODP Regulations (6100/6400)
- Experience in training or mentoring direct care staff
- Prior experience with electronic health record systems
