Summary: Responsible for the monitoring the health of the mothers, babies and children living at First Step Home Inc.
DUTIES AND RESPONSIBILITIES
- Responsible for meeting with newly admitted pregnant women and women with children to complete an initial assessment. Notify the medical and clinical team and others of any needs that are observed during the assessment.
- Responsible for arranging obstetric care for newly admitted maternal clients and pediatric care for children.
- Responsible for meeting weekly with pregnant women and children under one to teach/review: feeding, safe sleep, car seat safety, basic maternal health care. Checks baby’s weight and vitals each week during their visits. Meeting with all children over one year old at least once per month.
- Responsible for facilitating two maternal groups with a focus on fetal development during all trimesters, self-care during pregnancy and caring for baby after delivery etc.
- Collaborate with case management and the parent coordinator to connect women and children with existing services.
- Provide Care Coordination services by scheduling appointments for maternal women and children. Collaborates with PPC about appointments, missed appointments and follow up with high risk clinic.
- Responsible for completing Edinburgh assessments and reporting results to clinical and medical team.
- Responsible for completing grant requirements.
- Review all client summaries/discharge papers following visits from doctor’s appointments or hospital stays.
- Collaborates with FSH medical and clinical team for client referral for psychiatric assessment.
- Collaborates with medical and clinical team with developing trauma informed birth plans.
- Collaborates with clinical and medical team with supporting our clients to advocate for themselves when they are in the hospital, to ensure they are receiving fair and respectful treatment.
- The ability to communicate appropriately with outside agencies and maintain professional relationships.
- Works to attain the goals as written job performance evaluations.
- Abide by the standards as set forth in First Step Home’s Compliances Plan and Code of Conduct.
- Responsible for following all safety rules and safe health practices of First Step Home, Inc.
- Comply with all agency policies, procedures and directives of immediate supervisor, the President/CEO and the Board of Trustees of First Step Home.
- Responsible for following the Core Values of First Step Home.
- Other duties as assigned.
Qualifications:
- Graduate of an accredited school of nursing and holder of current state license. LPN or RN
- Behavioral Health experience
- CPR certified.
- CDCA or obtain within 90 days
- Minimum of six month of recent experience as a professional nurse.
Physical Requirements:
- Ability to perform prolonged or extensive standing and walking.
- Prolonged periods sitting at a desk and working on a computer.
- Ability to lift and/or pull 25 pounds frequently.
- Regularly exposed to the risk of bloodborne diseases.
- Regular exposure to unpleasant elements (accidents, injuries, and illness)
Qualifications
