Position Qualifications:
- MD LMSW or higher
- Inpatient Social Work experience
- Discharge planning/case management experience required
- EPIC experience
***Full time, every weekend, 3- 12 hour shifts, 8am to 8:30pm, Weekend differential and night time differential up to 22%

- Yearly (Based on Experience): $73,000 to $85,000
Responsibilities: Early Identification, high risk screening, and readmission screening of all patients. Services include: adjustments to illness counseling, safety assessments, discharge planning, identification of obstacles and barriers to discharge through proactive screening and assessments and coordination with the multi-disciplinary team.
