Position Overview
Pure Hearts NY, LLC is seeking a licensed Physical Therapist (PT) to provide school-based services to students with physical and mobility challenges. The Physical Therapist will work collaboratively with educators, related service providers, and families to support students in accessing their educational environment safely and effectively.
This role focuses on improving functional mobility, strength, coordination, and independence to enhance participation in academic and school-related activities.
Key Responsibilities
- Provide individual and/or group physical therapy services in accordance with students’ IEP mandates
- Conduct evaluations and re-evaluations to determine student needs and therapy goals
- Develop and implement evidence-based treatment plans aligned with educational objectives
- Collaborate with teachers, occupational therapists, speech therapists, and school staff
- Document services, progress notes, and reports in compliance with DOE/School District and NYSED requirements
- Participate in IEP meetings, case conferences, and parent communications
- Recommend and support adaptive equipment and mobility aids when appropriate
- Ensure compliance with all state, federal, and district regulations
Qualifications
- Active New York State Physical Therapist License (required)
- Experience in pediatric and/or school-based settings preferred
- Knowledge of IEP processes and special education services
- Strong clinical, communication, and documentation skills
- Ability to work independently across multiple school sites (if applicable)
