Position exemplifies service excellence in providing routine case management, skills training, medication training and support, crisis interventions and psychosocial rehabilitation services based in trauma informed and person driven care model. Maximize resources to enhance Lifepath Systems operations and support client’s strengths and needs, and helping to navigate medical, self-help, social, vocational, financial, educational, and other individualized services. Demonstrates integrity in role and by ensuring state standards and Lifepath Systems policies and procedures are met (i.e., timeliness, documentation, case management). Work independently with integrity and collaborative as a team member and community builder. Continuously improve in measurable ways on how to support individuals in service, community, and LifePath Systems services as a leader in the QMHP-CS role.
Education, Training, & Experience (including licensure & certification)

Bachelor’s degree in psychology, social work, or related field from a four-year college or university.
- At least two years of experience in working with the community, preferred. (i.e., social services, recovery, customer service, sales).

Bilingual (Spanish/English) preferred.
Knowledge, Skills, & Abilities
- Knowledge and experience in providing case management, skills training, and symptom management services to individuals with mental illness.

Ability to assess individual needs, to coordinate services, and to communicate effectively.
- Knowledge of the social services system and the ability to connect individuals with appropriate services.
- Ability to write clearly and professionally.

Ability to think clearly and logically in order to deal with potentially dangerous and/or emotional situations.
