Job Summary

Responsibilities
- Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts when necessary.
- Monitor patients’ health by taking vital signs, observing changes in condition, and reporting findings to healthcare professionals.
- Prepare nutritious meals tailored to individual dietary needs while maintaining a clean and safe kitchen environment
- Following care plans and HIPAA regulations to ensure patient safety.
- Support clients with mobility challenges by using assistive devices and providing safe transfer assistance.
- Maintain detailed documentation of care provided, patient observations, and any incidents in compliance with care plans and privacy standards.
- Engage in light housekeeping tasks including cleaning living areas to promote a healthy environment for clients.
Requirements
- MUST HAVE A VALID DL AND RELIABLE TRANSPORTATION WITH INSURANCE.
- Proven experience in home & community care or assisted living environments; group home experience is a plus.
- Knowledge of patient observation techniques, and health changes monitoring.
- Ability to lift heavy objects safely and perform physically demanding tasks involving resident transfers or mobility assistance.
- Able to work with individuals with developmental disabilities, Alzheimer’s disease, dementia, autism spectrum disorder, or memory care needs.
- Strong communication skills to collaborate effectively with healthcare teams and family members.
