Job Summary
Join our dedicated team as a PRN (as-needed) In-Home Aid, CNA (Certified Nursing Assistant), or PCA (Personal Care Assistant), where you will play a vital role in providing compassionate, personalized care to individuals in their home environments. Duties
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
- Monitor patient condition through patient observation, vital signs assessment, and reporting changes promptly
- Support clients with memory care needs such as Alzheimer’s and dementia care, ensuring a safe environment
- Utilize Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems for accurate documentation of care provided
- Operate assistive devices like Hoyer lifts and perform safe transfers to prevent injury
- Prepare meals, assist with light housekeeping tasks such as cleaning and laundry to maintain a hygienic living space
- Strong patient observation skills with the ability to recognize changes in condition or behavior
- Experience working in assisted living facilities, nursing homes, or home healthcare settings with geriatric patients or individuals with disabilities
- Knowledge of medical terminology, medication administration, and basic life support (CPR & First Aid) procedures
- Familiarity with hospice care, Alzheimer’s/dementia/memory care practices, and working with individuals with developmental disabilities
- Ability to operate medical equipment such as Hoyer lifts and perform catheter care safely
- Excellent communication skills for working effectively with clients, families, and healthcare teams while maintaining HIPAA compliance
- Physical capability for heavy lifting, assisting with transfers, and providing hands-on caregiving in home environments
