About the Role
As a Physical Therapist, you will play a pivotal role in enhancing your patients’ quality of life through individualized treatment plans. You’ll evaluate, plan, and implement effective therapy strategies while working collaboratively with a dynamic interdisciplinary healthcare team.
What You’ll Do
- Conduct comprehensive evaluations of patients’ functional abilities and limitations
- Develop and implement personalized, evidence-based care plans
- Provide therapeutic interventions including orthopedics, geriatrics-focused care, and trigger point therapy
- Collaborate with multidisciplinary teams for coordinated care and discharge planning
- Educate patients and caregivers on home exercise programs and lifestyle modifications
- Maintain accurate and timely documentation using EMR systems
- Deliver care across home health settings, with opportunities to engage in acute, outpatient, and community-based care
Basic Qualifications
- Strong knowledge of anatomy, physiology, and physical therapy principles
- Proficiency in medical terminology and clinical communication
Preferred Qualifications
- Experience in inpatient and/or outpatient settings
- Familiarity with home health and community-based care
- Background in geriatrics and/or pediatrics
