Responsibilities
- Assist patients with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility support using patient transfer methods and equipment such as Hoyer lifts.
- Monitor and document vital signs, medication administration, and changes in patient condition using Electronic Health Record (EHR) systems and medical documentation protocols.
- Provide specialized care for individuals with dementia, Alzheimer’s disease, or other memory impairments, including memory care and behavior management strategies.
- Support patients with medical procedures such as catheter care, wound care, and end-of-life care while maintaining strict adherence to home healthcare regulations and HIPAA confidentiality standards.
- Assist with meal preparation, light housekeeping tasks like cleaning and laundry, and ensure a safe environment for clients by following safety protocols including first aid and basic life support (BLS) procedures.
- Communicate effectively with clients, family members, and healthcare teams to coordinate care plans and update medical records accurately.
- Support clients during transfers or mobility exercises using proper patient transfer methods to ensure safety for both patient and caregiver.
Skills
- Proven experience working with individuals with disabilities or in assisted living settings; previous geriatric or hospice care experience is highly valued.
- Strong knowledge of medical terminology, medication management, vital signs monitoring, and medical documentation practices.
- Familiarity with EMR/EHR systems for accurate record keeping; understanding of home health care assistance regulations is essential.
- Excellent patient observation skills to detect changes in condition or behavior promptly; experience working with patients with dementia or developmental disabilities is preferred.
- Ability to perform heavy lifting safely when necessary; experience operating equipment like Hoyer lifts or other mobility aids is advantageous.
- Compassionate caregiver with excellent communication skills; committed to maintaining patient confidentiality per HIPAA standards.
- Certifications such as CPR, First Aid, Basic Life Support (BLS), and training in hospice or end-of-life care are desirable but not mandatory; ongoing training opportunities are available.
