Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using proper patient transfer methods such as Hoyer lifts and manual handling techniques.
- Monitor patients’ vital signs, observe changes in health status, and accurately document patient information in compliance with HIPAA regulations.
- Prepare nutritious meals tailored to individual dietary needs and assist with feeding as necessary.
- Administer medications following prescribed care plans and ensure proper medication administration procedures are followed.
- Support clients with memory care needs such as Alzheimer’s disease and dementia by implementing specialized care strategies and maintaining a safe environment.
- Provide transportation assistance for appointments, errands, or community activities to promote social engagement and independence.
- Maintain a clean and organized living or care environment through regular cleaning tasks while adhering to health standards.
- Communicate effectively with clients, family members, and healthcare teams to coordinate personalized care plans and ensure consistent quality of care.
Requirements
- Proven experience working with individuals with disabilities, neurodevelopmental disorders, or in senior/geriatric care settings such as assisted living or nursing homes.
- Knowledge of patient observation techniques, patient monitoring tools, and first aid/CPR certification.
- Familiarity with specialized care for Alzheimer’s disease, dementia, hospice patients, and memory care environments.
- Ability to perform heavy lifting safely using appropriate patient transfer methods like Hoyer lifts or manual handling techniques.
- Experience working within home health or community care environments is highly desirable.
- Strong communication skills for effective client interaction and documentation of care activities.
- Valid training in HIPAA compliance to ensure confidentiality of patient information.
- Flexibility to work various shifts including evenings or weekends as needed to provide consistent support.
