This position is located in Community Long Term Care (CLTC), Orangeburg County.
This is an in-office role and not a telecommute or remote position.
Are you the One? We are looking for an Eligibility Specialist I (CLTC) who works under general supervision of the Area Administrator. Provides technical support and assists the nurse consultant staff by performing pre-admission screening functions for applicants and participants seeking assistance from various CLTC (Community Long Term Care) programs. Assists with administrative support for the members of the nurse consultant and social work staff, vendors, provider case managers, and others as needed including assisting other CLTC offices with essential job functions according to program needs. Maintains a working knowledge of all CLTC programs and related systems including Phoenix information system.

Conducts referral processing activities.

Performs paraprofessional duties.

Promotes collaboration and effective communication.

Provides cross coverage assistance.

Processes Memorandum of Agreement (MOA) Pre-admission Screening Resident Review (PASRR).

Performs emergency preparedness duties or activities as required.

Performs other duties as assigned.
