Minimum of Bachelor’s Degree in Social Services / Social Work from an accredited college or university and have at least three years of paid, full-time experience in developing and/or implementing behavioral support and/or risk reduction strategies or plans. Must have a valid driver’s license with acceptable background checks. Must be able to apply for and maintain ODoDD Service and Support Administration certification.
Summary of Duties: Provide leadership to complete SSA responsibilities including individual service plans, monitoring, targeted case management and emergency crisis intervention. Ensures individual plans and related documents are completed timely and accurately and in compliance with applicable requirements. Establish and maintain a good working relationship with community organizations and service providers. Demonstrates skills in facilitation team meetings, including those that may be adversarial in nature, to reach agreement on needed services and supports for individuals. Maintains proficiency with AAI, ODDP, CPT and other tools needed for the effective implementation of the Medicaid waiver reimbursement system. Responsible for formal and informal orientation of new SSA and conducts trainings for new hires. Attend department meetings. Conduct HRC meetings and ensure HRC members have completed necessary trainings. Attend monthly behavior support workshop. Conduct assessments and develop behavioral support strategies that include restrictive measures. Complete Behavioral Assessments as needed. Ensure Restrictive Measure Notification (RMN) has been completed timely. Assist the SSA Director with submitting PAWS. Assist SSA with any billing issues they may have. Maintain a maximum caseload of twenty-five. 24 Hour On Call – Emergency Cell phone rotation. Complete service training and documentation in a timely manner. Complete all agency referrals in a timely manner. Make recommendations for referrals to community agencies and provide follow-up. Perform 24-hour crisis intervention services as scheduled.
