Education & Requirements
- Education: A Bachelor’s Degree from an accredited college or university is Required.
- Background Check: This is a sensitive position; an annual criminal background check is required.
- Transportation: Must have reliable transportation for frequent travel (at times over 50%) within Jasper County and occasional travel outside the county.
- Experience: Social work or case management experience preferred, but not required.

Job Summary
This is a beginning-level professional case management position. As a Support Coordinator I, you will manage a caseload of moderate size and complexity, focusing on connecting, planning, and advocating for service improvements for all persons served. You will work as part of an interdisciplinary team to determine proper treatments and navigate community resources.
Key Responsibilities
- Case Management: Develop and maintain Individual Service Plans (ISP) that respect the values and ideals of the person served.
- Operational Compliance: Meet organizational logging goals with a mandated 72-hour window for documentation.
- Data Management: Maintain accuracy in information systems including CIMOR and Setworks.
- Monitoring & Budgeting: Ensure service authorizations and budgets are completed on time and meet all local, state, and federal standards.
- Collaboration: Participate in meetings with providers or stakeholders ensure individuals are supported and needs are met.
Knowledge, Skills, and Abilities (KSAs)
- Disciplinary Background: Knowledge of social work, psychology, special education, counseling, health care, or occupational therapy.
- Case Management: Introductory knowledge of case management methods, principles, and techniques.
- Analytical Skills: Ability to collect and analyze information to make informed decisions regarding service plans.
