*$2,000 RETENTION BONUS* The employee will receive half after completing 6 months of employment and the remaining half after 1 year of employment.
A case manager is responsible for referrals, needs assessment, care plan development, outreach, and implementing and tracking services for an individual. Must be willing to be trained to administer medications. This position will be discussed in more depth during the interview.
Requirements:
- Must possess at least a bachelor's degree.
- 1+ years of experience is preferred, but a new graduate will be considered.
- Current/valid non-expired driver's license. This position requires driving. To meet the driving requirements, you must be at least 21 and have an insurable driving record.
- Social security card
- To perform this job successfully, an individual should have knowledge and use of computer software and programs, such as the ability to demonstrate computer skills in Word, Excel, Outlook, and type 40+ wpm.
- You will be required to complete MAC (Medication Assistant Certified) training.
- Orientation is required for employment. There are 5 days of paid orientation held Monday-Friday between 8:30 a.m. and 4 p.m.
Schedule: Monday–Friday, 8AM–4PM
