Concept Rehab is hiring a Physical Therapist (PT) to join our therapy team in Johnston, Iowa. We are currently offering Full-Time, Part-Time, and PRN opportunities at our skilled nursing facility. As a Physical Therapist, you will evaluate and treat patients recovering from illness, injury, surgery, and hospitalization in a skilled nursing and post-acute rehabilitation setting. You'll work closely with physicians, nurses, occupational therapists, speech-language pathologists, physical therapist assistants, patients, and families to develop individualized treatment plans that maximize mobility, independence, and quality of life.

Evaluate patients within 24–48 hours of physician orders

Develop and implement individualized treatment plans

Provide skilled, patient-centered therapy interventions

Supervise and collaborate with Physical Therapy Assistants

Communicate effectively with physicians, nursing, families, and care teams

Participate in interdisciplinary and care planning meetings

Document evaluations, treatments, and progress accurately and timely

Support discharge planning and caregiver education

Participate in staff education and in-service training as appropriate
