Qualifications:
- Registered Nurse with 3 -5 years home health experience
- Knowledge of home health, state and federal policies and procedures
- Ability to supervise, guide and develop skills and performance of service personnel.
Responsibilities: Assures that a patient’s plan of care is executed as written.
- Assures that a reassessment of a patient’s needs is performed by the appropriate health care professional when there is a significant change in the patient’s condition, at the physician’s request or after hospital discharge.
- Plans and arranges for consultation needs of staff; prepares and maintains current policies and procedures which meet Medicare, Medicaid, state, etc. laws and implements such.
- Serves on QAPI/utilization review committee and emergency preparedness committee.
- Responsible for overseeing development of indicators with appropriate data collection, aggregation and analysis, taking action, and reporting results according to Agency’s QAPI plan.
- Ensures compliance with federal, state, local laws, and Agency policies in all patient care aspects of the Agency.
- Reports to Administrator, any accident, incident, lost articles or unusual occurrences.
- Oversees staffing and visit assignments.
- Participates in marketing and community outreach.
- Other duties as assigned by the Administrator.

Pay: $40.00 - $65.00 per hour
