Job Overview
We are seeking a dedicated and energetic Licensed Vocational Nurse (LVN) – Field Nurse to join our dynamic healthcare team. Duties
- Conduct thorough patient assessments, including vital signs, physical examinations, and health history documentation using Electronic Health Record (EHR) systems such as Epic or Cerner.
- Administer medications, injections, IV infusions, and perform catheterizations following strict aseptic techniques and HIPAA regulations.
- Provide specialized care for patients with complex needs such as Alzheimer’s disease, dementia, memory care, and those requiring hospice or palliative care.
- Assist with wound care, dressing changes, tube feeding, ventilator management, and other advanced nursing procedures in various settings including hospitals, nursing homes, and outpatient clinics.
- Educate patients and their families on health management strategies including medication administration, nutrition, pain management, and disease-specific protocols like infectious disease control or acute care.
- Collaborate with interdisciplinary teams to develop discharge plans, case management strategies, and coordinate services for patients with developmental disabilities or those requiring long-term community-based care.
- Maintain accurate medical documentation of patient interactions, treatments administered, and clinical observations while ensuring compliance with all regulatory standards such as NCQA or JCAHO.
Skills
- Proven experience with EMR/EHR systems such as Epic, Cerner, Athenahealth or eClinicalWorks for documentation and patient tracking.
- Critical care experience in settings like ICU or Level I Trauma Centers is highly desirable.
- Strong background in medical-surgical nursing, geriatrics, pediatrics or home & community care environments.
- Skilled in performing complex procedures including phlebotomy, venipuncture, sterile processing techniques, and specimen collection/processing.
- Knowledge of physiology, anatomy, ICD coding (ICD-9/ICD-10), CPT coding for procedures and diagnoses.
- Ability to manage airway emergencies using airway management techniques and ventilator support when necessary.
- Excellent patient assessment skills combined with the ability to educate patients on health coaching topics such as nutrition or chronic disease management.
