Job Summary
The Home Care Registered Nurse (RN) is responsible for providing comprehensive skilled nursing services to patients in their homes. The RN performs assessments, develops and implements individualized care plans, educates patients and caregivers, coordinates care with the interdisciplinary team, and documents all services in accordance with state and federal regulations.
Essential Duties and Responsibilities
- Conduct comprehensive nursing assessments and reassessments.
- Develop, implement, and update individualized plans of care.
- Administer prescribed medications, injections, and intravenous (IV) therapy.
- Perform wound care, dressing changes, and wound assessments.
- Provide catheter insertion, maintenance, and care.
- Manage feeding tubes, ostomies, and tracheostomy care.
- Perform medication reconciliation and patient education.
- Monitor vital signs and assess changes in patient condition.
- Draw blood and collect laboratory specimens as ordered.
- Administer diabetic care, including insulin education and blood glucose monitoring.
- Provide post-operative nursing care.
- Assess pain and implement pain management interventions.
- Educate patients and family members regarding disease management, medications, safety, and self-care.
- Coordinate care with physicians, therapists, case managers, and other healthcare professionals.
- Maintain accurate and timely electronic clinical documentation.
- Participate in case conferences and quality improvement activities.
- Respond appropriately to emergencies and changes in patient condition.
- Ensure compliance with HIPAA, OSHA, CMS, Medicare Conditions of Participation, and state regulations.

Qualifications
- Current, unrestricted Registered Nurse (RN) license in the state of practice.
- Graduate of an accredited nursing program.
- Current CPR/BLS certification.
- Minimum of one year of nursing experience preferred (home health experience preferred).
- Valid driver's license, reliable transportation, and current auto insurance.
- Ability to work independently and manage time effectively.
- Strong assessment, communication, and documentation skills.
- Experience with electronic medical records (EMR) preferred.
Physical Requirements
- Frequently stand, walk, bend, kneel, and lift up to 50 pounds.
- Travel to patients' homes throughout the assigned service area.
- Ability to assist patients with transfers and mobility when necessary.

Documentation Responsibilities
- Complete Start of Care (SOC), Recertification, Resumption of Care (ROC), and Discharge assessments.
- Document skilled nursing interventions during each visit.
- Update care plans based on patient progress.
- Report significant changes in condition promptly to the physician.
- Maintain complete, accurate, and timely records in the agency's EMR.
Pay: $60.00 - $80.00 per hour
