Summary:
Patient Placement Coordinators are the central point persons for facilitating processes on a daily basis to expedite patient movement between departments and facilities for bed availability and utilization of such among multi-disciplinary levels to ensure the prompt facilitation of resources available for patient placement. Work collaboratively with Case Managers, Nurse Managers, Charge and Resource Nurses, Emergency Department Staff/Physicians, Hospitalists/Physician Extenders, Patient Registration, Patient Transport, and Environmental Services. The overall goal of the above mentioned team is to provide a seamless transition for the patient and to positively contribute to the patient experience. The incumbent performs all duties and tasks in accordance with performance standards established for the job.
Responsibilities:
- Monitors bed availability and maintains accurate and current census.

Is responsible for assigning and facilitating all patient placements.

Acts as the central and pivotal leader for all major communications regarding patient placement.

Works in collaboration with pre-access, patient registration, housekeeping, physicians, clinical nurse managers, ED staff, clinical advisors, care managers and providers to identify bed availability, patient flow via the bed tracking system.

Insures that the use of the ‘Bed Board” and other Patient Placement software is used consistently throughout the organization.

Develops and maintains patient flow data and monitors timeliness of admissions/transfers/discharges.

Works collaboratively with clinical nurse managers to appropriately manage resources in relation to acuity and patient mix.

Communicates with clinical nurse managers, directors, and administration/AOC any and all problems associated with high census, bed availability and implementation of patient placement plans.

Facilitates timely resolution of complex situations involving competing, multiple admitting/transfer requests whether they are elective, emergent or post operative patients.

Evaluates bed assignment according to clinical indicators and staffing resources.

Prioritizes admissions and transfers.

Assigns beds to patients who are admitted electively, emergently, post-operatively or are transferred.

Negotiates alternate bed assignments with requesting provider when requested bed is not available.

Is familiar with level of care criteria and can collaborate with Case Managers regarding appropriate admission status.

Acts as intermediary with Escort and ESS staff in lieu of on site supervisors on off shifts.

Facilitates direct admissions from other health care facilities and physician offices and transfer of patients from and to other health care facilities in compliance with policy, state and federal regulations.

Serves as a liaison between admissions office, medical and nursing staff, the emergency department, surgical services, and the AOC.

Leads Bed Meetings, monitoring attendance and compliance with communication expectations.

Communicates with unit management to coordinate admissions/transfers/discharges.

Communicates with appropriate individuals on day shift and evening/night nursing supervisor advisors on evenings/nights/weekends/holidays related to concerns with high occupancy.

Communicates frequently with SC’s at SMH and SSO in order to place patients within Spohn Health System if capacity and capability does not exist at SSH.

Develops and maintains effective processes and information systems to facilitate efficient and appropriate admissions and transfers.

Develops and maintains communication pathways so that timely accurate patient admission/transfer is accomplished.

Measures and communicates appropriate statistics as needed.

Maintains bed management and throughput data base and participates in the creations of and sustains appropriate work flow processes related to patient flow.

Acts as one of Teletracking System Administrators.

Educational role regarding patient flow and discharge planning.

Provides education/support to managers, staff and medical staff on effective work flow utilization of resources to match needs of patients.

Facilitates process improvement by evaluation of patient flow with multidisciplinary team process.

Provides guidance in staff flexing in relation to patient mix and acuity.

Ensures the quality of professional services by updating knowledge, skill and continuing professional growth.

Obtains educational experiences to meet continuing education needs and to maintain clinical expertise.

Participates in professional activities and organizations.
-
Follows EMTALA rules & regulations for patient transfers.
-
All other duties as assigned.
Requirements:
