Position Overview
The Case Manager/Transportation at Vita Nova plays a critical role in supporting residents of the maternity home by providing consistent case management, advocacy, and guidance throughout their stay. This position is responsible for partnering closely with residents to develop individualized goals, implement life skills programming, and coordinate access to internal and community-based resources that promote stability, self-sufficiency, and healthy family outcomes.
Key Responsibilities
- Provide comprehensive case management services to residents in a maternity home setting including intake, assessment, goal development, ongoing support, and transition planning
- Support program operations through a Monday-Friday schedule, participation in an on-call rotation, and occasional evening availability to meet resident needs and ensure program coverage
- Develop, document, and regularly update individualized case plans that align with program requirements and resident goals, ensuring timely and accurate recordkeeping
- Coordinate and facilitate assistance to additional community resources, including healthcare providers, social service agencies, educational programs, and other support organizations
- Provide residents transportation in company-issued vehicles while maintaining safety and professionalism
- Maintain clear, professional communication and collaborative relationships with internal staff, volunteers, and external partners to ensure continuity of care for residents
- Address resident concerns, behavioral issues, or conflicts promptly and constructively, reinforcing accountability while supporting emotional regulation and growth
- Protect resident confidentiality and maintain compliance with organizational policies, ethical standards, and applicable regulations
- Facilitate or support group programming and resident activities as needed to meet program goals
- Contribute to a collaborative team environment by supporting colleagues, sharing relevant information, and participating in meetings and training
- Additional duties as assigned
Qualifications
- Bachelor’s degree from an accredited four (4) year college or university is required, preferably in Social Work, Family Studies, Healthcare, Child and Family Welfare, Addiction Recovery, or a related field
- Minimum 2-4 years of experience working in social services or a related human services environment
- Proven leadership ability including listening, problem-solving, and decision making
- Valid driver’s license and successful completion of a motor vehicle record (MVR) review
- Ability to sit, stand, walk, and lift up to 25 pounds
Knowledge, Skills, and Abilities
- Strong case management and documentation skills with the ability to manage multiple residents and priorities simultaneously
- Exceptional verbal, written, and interpersonal communication skills
- Demonstrated ability to build trusting relationships while maintaining professional boundaries
- Proven problem-solving and decision-making skills, particularly in emotionally complex and high-stress situations
- Ability to remain calm, objective, and effective when responding to resident crises or conflict
- Self-motivated with the ability to manage multiple projects
