Part-Time Nurse Practitioner (Adult/Geriatric Care)
We are seeking an experienced Nurse Practitioners with a strong background in Adult and Geriatric Medicine to join our growing practice on a part-time basis.
Responsibilities:
- Conduct post-discharge evaluations and transitional care visits
- Develop and implement treatment plans
- Provide follow-up visits and ongoing patient management
- Deliver high-quality, patient-centered care in patients' homes
- Coordinate care with physicians, specialists, and support staff
Requirements:
- Licensed Nurse Practitioner in New York State
- Experience in adult and geriatric patient care
- Strong clinical assessment and care planning skills
- Comfortable providing care in patients' homes
- Reliable transportation and willingness to travel within an assigned geographic area
- Excellent communication and documentation skills
Schedule:
- Part-time position
- 1 to 3 days per week
- Flexible scheduling

Compensation & Benefits:
- Competitive hourly rate of $100–$155 per hour, based on experience and qualifications
