Hospice Visit Nursing is highly clinical work.
You're not simply checking in.
You're continually asking:
What's changed?
Is this patient comfortable?
Are the medications working?

Your assessment skills and ability to anticipate needs are incredibly important.
You'll have the autonomy to evaluate situations in the field while having physicians, clinical leadership, and an interdisciplinary hospice team available when you need additional support.

There's no nursing station down the hallway.
Your stethoscope, assessment skills, experience, and ability to communicate become some of your most important tools.
You'll walk into homes and senior living communities where patients and families may be facing situations they've never experienced before.
They need someone who can bring clarity.
Maybe a medication needs to be adjusted.
Maybe symptoms are progressing.
Maybe the family needs education about what they're seeing.
Sometimes they simply need an experienced nurse to tell them, "Here's what's happening, and here's what we're going to do."
That confidence can completely change a family's experience.
What You'll Do
As a Hospice RN Visit Nurse, you will:
- Provide skilled nursing visits throughout the Eastern Twin Cities Metro.
- Perform comprehensive assessments and ongoing patient evaluations.
- Manage pain, dyspnea, nausea, anxiety, wounds, medications, and other symptoms.
- Recognize changes in condition and intervene appropriately.
- Help anticipate patient needs before symptoms escalate.
- Educate patients and caregivers about medications, disease progression, comfort measures, and what to expect.
- Coordinate medication and treatment changes with physicians and providers.
- Help patients remain safely and comfortably in their preferred environment whenever possible.
- Communicate closely with hospice aides, social workers, chaplains, facility staff, and other members of the interdisciplinary team.
- Maintain accurate and timely clinical documentation.
- Participate in an on-call and weekend rotation.

Hospice and Palliative Care
You already understand comfort-focused care and supporting patients through end-of-life transitions.
Home Health
You're accustomed to independent field nursing, managing your day, communicating with providers, and assessing patients in their own environments.
Oncology
Your experience with complex disease processes, symptom management, difficult conversations, and family education translates exceptionally well.
ICU and Critical Care
You've developed strong assessment skills and know how to recognize subtle clinical changes before they become major problems.
Emergency Nursing
You're comfortable walking into uncertainty, assessing quickly, prioritizing needs, and remaining calm when everyone else is worried.
Assisted Living, Memory Care and Long-Term Care
You understand geriatric patients, dementia, chronic disease, family dynamics, and the realities of caring for medically complex older adults.
Med/Surg and Community Nursing
You've built the clinical foundation, organization, communication, and patient education skills needed to succeed in the field.
What We're Looking For
- Active Minnesota Registered Nurse (RN) license.
- Strong nursing assessment and critical-thinking skills.
- Confidence managing symptoms and changes in condition.
- Ability to work independently in the field.
- Excellent patient and caregiver education skills.
- Strong communication and interdisciplinary collaboration.
- Compassionate but clinically focused approach to end-of-life care.
- Reliable transportation and willingness to travel throughout the Eastern Twin Cities Metro.
- Hospice experience preferred, but strong transferable nursing experience is encouraged.
