Job Overview
We are seeking a highly skilled and compassionate Advanced Practice Provider (Nurse Practitioner or Physician Assistant) for our Primary Care Team. This role involves delivering high-quality patient assessments, managing acute and chronic conditions, and supporting our mission to provide effective, affordable mental health and medical services within a community setting. This position is contract, seeking 8 to 16 hours of weekly coverage (Monday - Friday, no holidays and no weekends).
Key Responsibilities
- Provide comprehensive primary care services including assessment, diagnosis, and treatment of acute and chronic conditions
- Conduct physical exams and obtain medical histories
- Order and interpret laboratory and diagnostic tests
- Prescribe medications and treatments in accordance with state and federal regulations
- Provide preventive care including wellness exams, screenings, and immunizations
- Manage chronic diseases such as diabetes, hypertension, and asthma
- Educate patients on disease prevention, lifestyle modifications, and medication adherence
- Collaborate with physicians, behavioral health providers, dentists, and care coordinators in a team-based care model
- Document care accurately in the electronic health record (EHR)
- Participate in quality improvement initiatives and population health efforts
Qualifications
- Graduate of an accredited Nurse Practitioner (NP) or Physician Assistant (PA) program
- Current state licensure and national certification
- Current DEA license or eligibility
- Current BLS certification
- Strong commitment to serving medically underserved populations
- Experience in family medicine, community health, or primary care (preferred)
Work Location: In person
