Responsibilities
- Conduct comprehensive assessments to evaluate patients’ physical, cognitive, and emotional abilities, including assisting with activities of daily living (ADLs) and functional mobility.
- Develop and implement individualized care plans aligned with clinical goals, medical documentation standards, and evidence-based practices.
- Collaborate with healthcare professionals, educators, families, and caregivers to coordinate discharge planning and ensure seamless patient transitions across settings such as inpatient units, outpatient clinics, schools, or home health.
- Provide therapy interventions for individuals
- Assist patients with disabilities in improving motor skills, sensory processing, and adaptive techniques through hands-on therapy and activity modifications.
- Maintain accurate electronic health records (EHR) management and document all patient interactions using medical terminology clearly and thoroughly.
- Requirements
- Valid licensure or certification as an Occupational Therapist in California
- Proven experience working in skilled nursing facilities, outpatient clinics, or home health environments or related clinical experience.
- Strong knowledge of physiology and anatomy to inform effective treatment strategies.
- Familiarity with medical documentation standards, medical terminology, and electronic health records (EHR) management systems.
- Ability to work effectively within multidisciplinary teams on case management and discharge planning initiatives.
- Excellent communication skills
Pay: $65.00 - $75.00 per hour
