We're looking for a passionate and empathetic Health Coach who enjoys helping people make meaningful lifestyle changes and stay engaged in their healthcare journey.
Position Summary
As a Health Coach, you will work directly with Medicare patients through phone calls, text messaging, and virtual communication to encourage healthy behaviors, improve medication adherence, and support patients in achieving their health goals. You will serve as a trusted partner, helping patients navigate their care while working closely with nurses and care coordinators.
Key Responsibilities
- Conduct scheduled outbound wellness and coaching calls.
- Build trusting relationships with patients through compassionate communication.
- EducAssist patients in setting and achieving realistic health goals.
- Reinforce medication adherence and healthy lifestyle habits.
- Provide education on nutrition, exercise, stress management, sleep, and chronic disease self-management.
- Encourage patients to complete daily RPM readings (blood pressure, glucose, weight, pulse oximetry, etc.).
- Document all patient interactions accurately in the care management platform.
- Identify patients needing clinical escalation and notify the nursing team promptly.
- Coordinate with providers, caregivers, and internal care teams when appropriate.
- Support enrollment and ongoing engagement in CCM, RPM, BHI, and CHI programs.
- Maintain HIPAA compliance and patient confidentiality at all times.
QualificationsRequired
- High school diploma or equivalent.
- Excellent verbal and written English communication skills.
- Strong customer service and relationship-building abilities.
- Comfortable speaking with elderly patients over the phone.
- Strong organizational and time management skills.
- Reliable high-speed internet and quiet home office.
- Basic computer proficiency.
Preferred
- Bachelor's degree in Health Sciences, Public Health, Nursing, Nutrition, Psychology, Exercise Science, or related field.
- Health coaching certification (NBHWC or equivalent) is a plus.
- Experience in chronic disease management or care coordination.
- Experience working with Medicare populations.
- Bilingual (English/Spanish) is a plus.
- Experience using EHR/EMR or care management software.
