Job Overview
We are seeking an experienced and compassionate Registered Nurse (RN) to join our hospice team as a Per Diem Case Manager serving Stockton/Lodi/Manteca and surrounding communities. This role is ideal for a nurse who values patient-centered care, autonomy, and meaningful connections with patients and families during end-of-life care. In this vital role, you will deliver holistic nursing care, coordinate interdisciplinary services, and educate families to ensure a peaceful and meaningful experience for patients and their loved ones.
Responsibilities
- Conduct comprehensive patient assessments including vital signs, symptom management, and physiological evaluations to develop individualized care plans.
- Administer medications and treatments such as IV infusions, pain management protocols, and medication administration aligned with hospice standards.
- Collaborate with interdisciplinary teams to coordinate services like palliative therapies, psychological support, and spiritual care that enhance patient comfort.
- Educate patients and families on disease progression, symptom control techniques including ostomy care or tube feeding, and advance directives to facilitate informed decision-making.
- Document all patient interactions accurately using electronic health record (EHR) systems such as Epic or Cerner while maintaining compliance with HIPAA regulations.
- Provide emotional support and counseling to patients experiencing acute pain or distress, ensuring compassionate end-of-life care.
- Participate in ongoing training on topics like hospice & palliative medicine, infectious disease care, and trauma medicine to stay current with best practices.
Qualifications
- Valid nursing license with current registration in the state of California
- Proven experience in hospice & palliative medicine or related fields such as critical care, hospital medicine, or home care.
- Familiarity with EMR/EHR systems including Wellsky/Kinnser for accurate medical documentation.
- Ability to handle complex cases involving acute pain management, dementia or Alzheimer's care, behavioral health issues, and working with individuals with disabilities or developmental delays.
