Operating as a commission-only sales professional, this role focuses on engaging hospital discharge planners, social workers, health systems, physician practices, and care coordinators to transition patients seamlessly into high-quality private duty home care. 1. High-Impact Outreach & Relationship Building
Identify, target, and cultivate strong professional relationships with hospital case managers, discharge planners, physicians, outpatient clinics, and rehabilitation specialists.
Position the agency as the preferred non-medical home care partner to streamline patient discharge planning and reduce hospital readmission rates.
Conduct frequent in-person field visits, clinical presentations, and consultation sessions with healthcare providers to secure ongoing client referrals.
2. Referral Conversion & Client Admissions
Actively convert medical referrals into start-of-care contracts by following up promptly with prospective clients and their families.
Perform initial client assessments and consultations, communicating care plans clearly to families while closing service agreements.
Collaborate with internal care coordination and staffing teams to ensure rapid admission turnarounds and zero friction during patient discharge.
3. Territory Management & Revenue Growth
Develop and execute a targeted territory growth strategy to maximize lead generation, active account development, and client conversion rates.
Track and report all physician and hospital outreach, pipeline activity, and closing metrics in the company CRM.
Continuously monitor market trends and local hospital system shifts to capitalize on new business opportunities.
Experience: Minimum 1 year of proven healthcare sales, pharmaceutical sales, or hospital liaison experience—preferably within private duty home care, home health, or senior care.
Track Record: Demonstrated ability to open new doors, close sales, and thrive in performance-based or commission-driven environments.
Healthcare Knowledge: Strong grasp of hospital discharge workflows, physician referral habits, private pay options, and long-term care insurance.
