Essential Responsibilities - LPN (Licensed Practical Nurse)
As a valued LPN, you will provide compassionate, high-quality care while supporting resident wellness, safety, and dignity in a collaborative long-term care environment.
✔ Provide direct nursing care and implement individualized resident care plans
✔ Administer medications, treatments, and pain management interventions safely and accurately
✔ Monitor resident health status, including assessments, skin integrity, wound care, and vital signs
✔ Observe, assess, document, and promptly report changes in resident condition
✔ Execute physician orders accurately and within required timeframes
✔ Maintain accurate electronic health record (EHR) documentation and clinical charting
✔ Ensure compliance with infection control protocols and all State and Federal healthcare regulations
✔ Respond effectively to urgent and emergent resident care situations using sound clinical judgment
✔ Communicate clearly and professionally with residents, families, physicians, and interdisciplinary care teams
✔ Lead, delegate, and support CNA and clinical support staff while fostering teamwork and accountability
✔ Assist with staffing coverage and provide unit support as needed
✔ Deliver comprehensive shift reports to ensure continuity of care
✔ Provide exceptional resident-centered customer service by exceeding resident and family expectations
✔ Float between units based on operational and resident care needs

Education, Training, Experience
✔ High school diploma or GED required
✔ Graduate of an accredited Licensed Practical Nurse (LPN) nursing program
✔ Successful completion of State Board of Nursing licensure examination
✔ Previous LPN/RN nursing experience preferred, ideally in long-term care, skilled nursing, rehabilitation, assisted living, or senior care.
✔ Experience working with geriatric or elderly populations preferred
✔ Must maintain all continuing education requirements to keep nursing licensure active and in good standing
✔ Participates in ongoing learning, professional development, and required in-service training
Required Licenses, Registration, and/or Certifications
✔ Current, active Michigan Board of Nursing License as a Licensed Practical Nurse (LPN)
Physical Requirements
This role is active and hands-on. You must be able to:
✔ Stand, walk, bend, stoop, lift, push, and pull throughout the shift
✔ Assist residents with mobility and activities of daily living as needed
✔ Safely lift up to 50 pounds
✔ Operate mechanical lifts and clinical equipment safely and effectively
✔ Maintain visual acuity and physical stamina necessary to deliver quality resident care
