Summary of Duties
The Social Work Manager is responsible for the St. Francis Hospice and supportive services Psychosocial and Bereavement programs. This position leads the social work team in providing high quality assessment and appropriate intervention to patients and families receiving hospice and bereavement care. The Social Work Manager provides leadership to interdisciplinary teams regarding the psychosocial and bereavement needs of patients and their families. Furthers the knowledge and understanding of the social work role of Hospice and Bereavement through appropriate educational outreach.
- Oversees all aspects and components of the Social Work and Bereavement Programs to ensure that the highest quality of support is provided safely and efficiently following hospice regulations and St. Francis guidelines
- Compiles, analyzes, submits and maintains regularly scheduled reports related to hospice and bereavement programs to include, but not limited to, statistical summaries, analyses, and evaluations

Identifies and examines trends and problems. Develops solutions as it relates to the psychosocial aspects of hospice and bereavement care

Develops and revises policies, procedures, and processes for the psychosocial aspects of the hospice and bereavement programs

Maintains costs within budget
- Effectively listens to and communicates with internal and external customers, ensuring that essential information is communicated on a timely basis

Plays a leadership role in hospice and bereavement events

- Liaises with professional associations, partners and social work resources to promote current standards of practice an excellence of care

Provides education to clinical partners, patients, families, and staff regarding the St. Francis social work and bereavement programs

Oversees administrative functions, including correspondence with community for both the hospice and bereavement psychosocial programs

Represents the organization at community events to cultivate meaningful partnerships and strengthen community engagement. Events may occur outside of standard business hours
- Takes a leadership role in the development of the Patient/Family Interdisciplinary Plan of Care, with particular attention to emotional, social, spiritual and family needs

Provides the social work perspective at Interdisciplinary Group meetings. In cooperation with other disciplines, develops and maintains a plan of care which meets the needs of the patient/family

Collaborates with Hospice Managers on integration of clinical and psychosocial needs of patients and their families
- Ensures that all patient information follows HIPAA guideline

Prepares and participates in regulatory/accrediting surveys including the Department of Health and Joint Committee

Strong working knowledge of CMS CoPs (Conditions of Participation) for Hospice, NHPCO, and CAPC standards for quality and compliance

Ensures documentation, service delivery, and team operations meet or exceed regulatory requirements

Adheres to and complies with St Francis code of ethics, policies and procedures

Adheres to safety, health, and regulatory requirements as described in the administrative and departmental policies and procedures. Reports incidents and unsafe work conditions
Patient Care
- Provides hospice and bereavement social casework and counseling as needed
-
Evaluates the psychosocial needs of patients/families, taking into consideration developmental history, current family system, source and adequacy of income, medical insurance, need for services or community referrals, postmortem arrangements, mental status and coping capacity, patient's and family’s reaction to illness, bereavement coping and role impairment

Maintains accurate and complete patient records, following company standards
- Leads and mentors team, providing guidance and support on educational and developmental opportunities relevant to the position

Establishes and drives a healthy team environment. Proactively addresses employee safety, morale and operational concerns

Evaluates the performance of programs and recommends solutions to improve performance, when needed

Responsible for interview, selection, orientation, training, performance management, and transfer/promotion of direct reports. Initiates disciplinary action and termination as appropriate in accordance with relevant policies, collective bargaining agreements, and job expectations

Monitors and ensures staff productivity, including schedules, overtime, time off and leaves, and absenteeism. Reviews and approves timesheets

Identifies opportunities to improve performance or enhance collaboration

Participates in inter-departmental committees, teams and task forces as needed

Ensures staff competencies are completed within designated timeframe

Promotes professional development opportunities for team
Education/Training
-
Bachelor's degree in social work required. Master’s degree in social work preferred
Licensure and Certifications
-
Current licensure as a Licensed Clinical Social Worker (LCSW) in the State of Hawaii
Experience
- 5 years of experience in increasingly responsible work in social work, preferably hospice or bereavement services. Minimum of 2 years of hospice experience preferred

Previous supervisory experience required, preferably in a hospice environment

Knowledge of the policies, procedures and regulations of social work programs

Knowledge of state and federal social welfare and hospice laws, rules and regulations

Strong understanding of Medicare and Medicaid guidelines
-
Excellent verbal and written communication skills, with ability to effectively listen to clients, family members, staff, vendors and contractors

Excellent customer service and interpersonal skills with ability to be patient, maintain composure and interact with clients, family and staff with courtesy, tact and discretion

Documented computer skills

Ability to both organize and prioritize work

Ability to be flexible and adapt to changing situations on short notice with ease

Ability to safely operate an automobile. Must have personal automobile available for business use, valid/current driver’s license and good driving record as evidenced by current traffic abstract

Regular contact with co-workers, other departments, and persons and organizations outside the St. Francis Healthcare System. Requires use of tact and discretion. May present data and reports at meetings

Must be able to listen/understand what the client/client family needs are and clearly communicate with the client and their family. Job requires employees to communicate in English while in public/common areas where conversations can be overheard by others
