TANDEM365 is looking for a compassionate and motivated Medical Assistant to join our growing team in Wayne County. This role is ideal for someone who enjoys helping others, building relationships, and working closely with participants to improve their health and quality of life.
As a Medical Assistant, you will work alongside nurses, social workers, community paramedics, to help participants access providers, community partners, healthcare services, coordinate appointments, navigate community resources, and stay connected to the support they need. Our team focuses on helping individuals with complex medical, behavioral health, and social needs remain healthy and safely at home.

What You'll Do
- Connect with participants by phone and in their home
- Help schedule appointments and follow-up care
- Follow up with participants after hospital or emergency department visits
- Assist with care coordination and communication with healthcare providers
- Connect participants to community resources and services
- Provide support and reinforce health education provided by the care team
- Communicate with participants, caregivers, providers, and community partners
- Participate in team meetings and care planning
- Maintain accurate and timely documentation
- Help participants with medication set up
- Draw participants labs in their home
What We're Looking For
- Graduate of an accredited Medical Assistant program
- Medical Assistant certification preferred
- Strong communication and organizational skills
- Ability to work independently and as part of a team
- Comfortable using technology including Microsoft and electronic medical records
- Valid Michigan driver's license, reliable transportation, and automobile insurance
- Responsible to drive anywhere in Wayne County
