All Well-Being Services (AWBS) is a CARF-accredited non-profit organization and DWIHN provider, proudly serving the Wayne County community for over 70 years. We offer comprehensive, person-centered services to individuals with mental health conditions, substance use disorders (SUD), and intellectual and developmental disabilities (IDD). Our programs include case management, therapy, transportation, skill-building, and more, all designed to promote independence, well-being, and an improved quality of life. Position Overview
The Substance Use Disorder (SUD) Case Manager plays a vital role in assessing, coordinating, and monitoring mental health services for individuals living with SUD. This position involves developing Individualized Plans of Service (IPOS), connecting clients to resources, and ensuring access to necessary behavioral health supports.
The SUD Case Manager collaborates with clients, families, service providers, and community agencies to advocate for client needs, enhance stability, and support recovery-focused outcomes.
*We are currently looking for full-time and part-time (evenings and weekends) for this position.
Key Responsibilities
- Develop and implement Individualized Plans of Service (IPOS) based on client needs and treatment goals.
- Provide comprehensive case management, ensuring clients have access to appropriate mental health and community resources.
- Conduct assessments and ongoing evaluations to monitor progress and make necessary adjustments to treatment plans.
- Advocate for client rights and access to services, helping individuals navigate the mental health system.
- Collaborate with therapists, psychiatrists, medical professionals, and community agencies to support client care.
- Assist clients with housing, employment, benefits, and other social service needs.
- Provide crisis intervention and support when necessary.
- Maintain accurate and timely documentation in compliance with agency and regulatory requirements.
- Educate clients and families about mental health conditions, treatment options, and coping strategies.
- Perform other duties as assigned by the supervisor.
Required Skills & Qualifications
- Bachelor’s degree in Human Services (e.g., Social Work, Psychology, Counseling, or a related field).
- Experience in mental health, case management, or social services preferred.
- Strong advocacy, problem-solving, and crisis intervention skills.
- Knowledge of community resources and mental health services.
- Excellent organizational, communication, and documentation skills.
- Ability to work both independently and as part of a multidisciplinary team.
- Valid driver’s license and reliable transportation required.
