EliteCareMDs Tampa LLC is seeking a dedicated and clinically skilled Nurse Practitioner (NP/APRN) or Physician Assistant (PA) to provide mobile pulmonary consultation services to patients in skilled nursing facilities (SNFs), assisted living facilities (ALFs), rehabilitation centers, and post-acute care settings throughout Ft. Myers, Ocala, Orlando, and surrounding areas.
This role focuses on bedside pulmonary evaluation and management of medically complex adult and geriatric patients. The ideal candidate is compassionate, efficient, collaborative, and comfortable practicing independently in post-acute environments.
Our mobile care model is designed to improve patient outcomes, reduce unnecessary hospitalizations, and provide specialty pulmonary care directly within facilities.
Key Responsibilities
Pulmonary Assessment & Management
- Perform comprehensive pulmonary evaluations and follow-up visits.
- Diagnose and assist in management of COPD, asthma, pneumonia, chronic respiratory failure, pleural effusions, ILD, pulmonary hypertension, and other pulmonary conditions.
- Manage oxygen therapy, inhaler regimens, nebulizer treatments, and non-invasive ventilation (CPAP/BiPAP).
- Evaluate shortness of breath, hypoxia, chronic cough, and abnormal imaging findings.
Post-Acute & Facility-Based Care
- Provide pulmonary consultations in SNFs, ALFs, acute rehab facilities, and long-term care settings.
- Collaborate with physicians, nursing staff, therapy teams, and specialists to optimize patient care.
- Assist with transitions of care and reduction of avoidable hospital readmissions.
Documentation & Care Coordination
- Maintain accurate and timely EMR documentation.
- Review laboratory data, imaging, hospital records, and medication profiles.
- Coordinate referrals, diagnostics, and follow-up care as needed.
- Educate patients, caregivers, and facility staff regarding pulmonary treatment plans.
Qualifications
● Active Florida APRN or Physician Assistant license required.
● Strong clinical assessment and bedside communication skills.
● Ability to work independently and efficiently in mobile clinical settings.
● EMR proficiency.
● Reliable transportation required.
● Strong communication and interdisciplinary collaboration skills.
● Comfortable working in mobile and fast-paced clinical environments.
● New graduates with strong clinical training and interest in pulmonary or post-acute care are encouraged to apply — willing to train the right candidate.
