Overview

Responsibilities
- Conduct thorough patient assessments and diagnostic evaluations to develop individualized care plans.
- Administer medications and treatments as prescribed, ensuring adherence to aseptic techniques.
- Monitor patient progress and respond promptly to changes in condition, providing appropriate interventions.
- Collaborate with interdisciplinary teams in case management to facilitate effective patient outcomes.
- Maintain accurate patient records and documentation in compliance with nursing home policies and regulatory standards.
- Educate patients and families on health management, treatment options, and preventive care strategies.
- Participate in quality improvement initiatives to enhance patient safety and care delivery.
Experience
- Current Registered Nurse (RN) license in the state of Ohio.
- Previous experience in a nursing home or hospital setting is preferred.
- Experience in senior care or working with geriatric populations is a plus.
- Strong communication skills, both verbal and written, with the ability to work effectively in a team-oriented atmosphere.
- Proficiency in using electronic health records (EHR) systems for documentation purposes such as Point Click Care.
