Duties
- Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using equipment such as Hoyer lifts and wheelchairs
- Monitor and document patient observations, vital signs, and changes in condition accurately within Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems
- Provide specialized care for patients with dementia, Alzheimer's disease, or other memory-related conditions, ensuring their safety and comfort
- Support medication administration following proper protocols while maintaining strict HIPAA compliance to protect patient privacy
- Perform catheter care, wound care, and other medical procedures as trained and authorized by nursing staff
- Assist with meal preparation, feeding, laundry, cleaning, and general household tasks to promote a safe and clean environment
- Collaborate with interdisciplinary teams to coordinate home & community care services tailored to individual needs
Qualifications
- Current certification as a Certified Nursing Assistant (CNA) or Licensed Nursing Assistant (LNA) required; hospice experience preferred
- Demonstrated experience working in hospice care, long-term care facilities, nursing homes, assisted living or home health settings
- Strong knowledge of medical terminology and familiarity with EMR/EHR systems for accurate documentation
- Proven ability to perform patient observation and assist with ADLs effectively in diverse settings including home & community environments
- Certifications in CPR and first aid; additional training in dementia care or Alzheimer’s care is highly desirable
- Experience working with individuals with disabilities or developmental challenges preferred
- Ability to handle sensitive situations with compassion while maintaining professionalism and confidentiality

Pay: $20.00 - $25.00 per hour
Expected hours: 3.0 – 10.0 per week
