Salary: Salary placement on our Certified Salary schedule is dependent on the employees' education and/or work experience. Certified/licensed employees will be granted one (1) year for each full year of contractual and/or work experience up to a maximum of ten (10) years, which includes in-district and out-of-district contractual teaching and/or work experience. EDUCATION/EXPERIENCE
Required:
-
Bachelor's degree in Nursing from an accredited institution of higher education
-
Valid Colorado Registered Nurse (RN) license
-
Eligible for or licensed by the Colorado Department of Education as required for school nursing practice

American Heart Association First Aid and CPR certification
-
Valid Colorado driver's license and ability to travel between assigned school and community sites
-
Eligibility for Instructor Certification in CPR and First Aid
Preferred:

Experience working with children, adolescents, families, schools, and/or community health systems
-
Experience working with interdisciplinary teams and community-based organizations
-
Experience with health equity, trauma-responsive care, culturally responsive practice, or addressing social determinants of health
-
Experience with data collection, program evaluation, or quality improvement

CPI training or willingness to obtain certification after hire.
JOB DESCRIPTION
Summary/Objective: The Community Schools Nurse Case Manager is a Registered Nurse and student advocate who collaborates with school leaders, families, healthcare providers, and community partners to eliminate health-related barriers to learning. Supporting the phased implementation of the District's Community Schools model, this role establishes proactive systems for care coordination, physical and mental health support, and emergency response. Through assessment, case management, and targeted advocacy, the Nurse Case Manager ensures all students have the resources needed to attend school safely and succeed academically. Additionally, the position drives program evaluation and continuous improvement by monitoring and reporting key data on student referrals, service utilization, and health outcomes.
Essential Job Functions:

Assesses Student Health: Evaluates physical, mental, and social health needs to create personalized care plans for students with chronic or complex medical conditions.

Provides Direct Care & Training: Delivers direct nursing care during health emergencies and trains school staff to safely handle medications and medical procedures.

Coordinates Services & Resources: Connects students and families to doctors, mental health providers, food assistance, and local resources to remove barriers to school attendance.

Partners with Families & Staff: Collaborates with parents, teachers, and healthcare providers�including making home visits when needed�to advocate for student safety and well-being.

Tracks Data & Maintains Records: Keeps accurate, confidential health records and tracks service data to help improve health programs across the school district.

Supervisory Responsibilities: Health Clerks
Work Environment/ Physical Demands: Ability to sit for long periods of time, numerous distractions, visual, auditory and ambulatory acuity important. Occasional lifting of up to 30 pounds.
Travel: Work occurs primarily in school settings and may include travel among assigned Community School sites, district offices, healthcare settings, community agencies, and student homes.
