What You’ll Do
- Build and grow referral relationships with:
- Physician offices
- Hospital discharge planners & case managers
- Skilled nursing facilities (SNFs)
- Assisted living communities
- Educate providers on home health eligibility and our services
- Generate qualified referrals that convert to admissions
- Conduct in-person visits and ongoing follow-ups with referral sources
- Track outreach, referrals, and conversion outcomes
- Work closely with intake and clinical teams to ensure fast and smooth patient admissions
What We’re Looking For
- Experience in home health, hospice, or healthcare marketing preferred
- Strong understanding of Medicare home health eligibility (or willingness to learn quickly)
- Existing relationships in the Lehigh Valley is a strong plus
- Highly motivated, independent, and results-driven
- Strong communication and relationship-building skills
- Reliable transportation for local travel
Compensation Structure
- Competitive hourly rate (based on experience)
- Per-Admitted Patient Bonus (Uncapped)
- Standard admission: $75 – $100
- High-value cases (e.g., wound care, multiple disciplines, higher utilization): $125-$175
- Monthly performance incentives for consistent admissions
Bonuses are paid on confirmed admissions, not just referrals—rewarding quality over volume.
