Job Overview
We are seeking an experienced, certified Adult Nurse Practitioner (APRN/NP) to join our established primary care practice in West Hartford.
The ideal candidate will have prior adult primary care experience and be comfortable independently evaluating and managing adult patients, diagnosing and treating common medical conditions, ordering and interpreting appropriate testing, prescribing medications, and providing preventive and ongoing primary care.
Duties
- Conduct thorough physical examinations and patient assessments across various age groups, such as geriatrics, and adult populations.
- Prescribe medications and treatment plans in accordance with collaborative practice agreements and regulatory guidelines such as HIPAA and DEA regulations.
- Independently evaluate, diagnose, treat, and manage primary care patients within scope of practice
- Manage chronic conditions including hypertension, diabetes, hyperlipidemia, obesity, asthma, and other primary care conditions
- Utilize electronic health record (EHR) eClinicalWorks for accurate medical documentation, medication administration records, and medical records management.
- Provide preventive care, health screenings, immunizations, and patient education
- Review and follow up on laboratory and diagnostic results
- Coordinate referrals and collaborate with specialists when appropriate
Skills
- Skilled in medication administration including injections and CPT coding for billing purposes; familiar with ICD-9/10 coding systems for diagnoses documentation.
- Demonstrate ability to manage patients independently with minimal day-to-day supervision
- Experience in telehealth platforms to provide remote consultations while maintaining high-quality patient care standards.
- Excellent client communication skills with the ability to educate patients on treatment plans.
- Dependable, organized, and capable of managing a busy outpatient schedule.

Pay: $111,757.21 - $130,000.33 per year
